No referral is necessary, but if you have an HMO and need an insurance authorization from your primary care physician, please arrange this before your appointment with us. If you have insurance questions, please call our billing office at (541) 282-6687.
MAP & DIRECTIONS
Coming from the South:
Take I-5 north to the Medford, Barnett Road exit #27. Turn right onto E Barnett Road. Travel approximately one mile. Turn left at Medical Center Dr. (This is the entrance to the Rogue Valley Medical Center campus.) Continue on Medical Center Dr. past the RVMC Emergency Department ramp. The Advanced Vein Center is located on the third floor of the Cardiovascular Institute Building which will be on your left just past the Smullin Health Education Center.
Coming from the North:
Take I-5 south to the Medford, Barnett Road exit #27. Turn right onto E Barnett Road. Travel approximately one mile. Turn left at Medical Center Dr. (This is the entrance to the Rogue Valley Medical Center campus.) Continue on Medical Center Dr. past the RVMC Emergency Department ramp.
